Make a Referral to Youth Home
Youth Home accepts referrals from mental health professionals, educators, physicians, and social service agencies. If you believe a youth would benefit from our services, we encourage you to use our referral form to get started.
Referrals should include :
- Medical necessity documents
- Clinical insight (when appropriate)
- Age of client
- Gender of client
- Function / development level of client
- Diagnosis
- History
